Champions for Inclusive Communities

Champions for Inclusive Communities is a 4-year cooperative agreement between the Maternal and Child Health Bureau and the Early Intervention Research Institute at Utah State University. The focus on Champions InC is to support states and communities in implementing community-based systems of services and supports for CYSHCN and their families.

What Was Champions for Progress?

The Champions for Progress Center provided leadership support for state and territorial Title V programs in the process of systems building at the state and community levels for children and youth with special health care needs (CYSHCN). The Champions web portal is still available for anyone involved with CYSHCN to link states and communities in their efforts to create community-based systems of care for CYSHCN.

Focus on Progress

Champions for Progress now offers a concise "snapshot" of each state's system of care for CYSHCN called Focus on Progress.

The Focus on Progress provides a quick reference for sharing information with families, state and community programs, legislators, and other partners. Stay tuned as each state's Focus on Progress is added to the Champions website when it is completed.
