Tools for building a system of care to achieve the Performance Measures
Step 1: Building Partnerships |
|
Why Is This Important?Building partnerships is important because no one program or agency can provide all the services and resources needed by CYSHCN and their families. State CSHCN leaders must work with families to identify the unique public and private entities that make up the system of care. Building interactive partnerships with these entities will help to assure that the state's system is looking out for children using the broad definition of children and youth with special health care needs. Building partnerships is the first step in the systems building process for establishing a system of care for CYSHCN. Building Partnership Tools |
