Why Is This Important?The natural outgrowth of a state plan for CYSHCN that is built upon public/private partnerships should be a community-based system of care where services are organized so that families can use them easily. Models of interagency planning and implementation suggest that communities assess and build upon their strengths and receive support and guidance via a unifying state plan.
