State Incentive Awards

Over the last three years 34 states and territories, including the Navajo Nation, have received Incentive Awards from the Champions for Progress Center through a competitive application awarding process.

Funds were allocated by Champions to be offered to states in order to support and strengthen state efforts to address community-based systems of care.

The activities conducted as part of each incentive award project were tied to achieving the CHSCN Performance Measures delineated in the State Title V Block Grant and state strategic plan for achieving a system of care for children with special health care needs.
