Tools for building a system of care to achieve the Performance Measures
Step 4: Measuring & Monitoring |
|
Why Is This Important? The six CSHCN performance measures now offer states a standard set of goals for children and youth with special health needs care and their families. In addition to using data from the National Survey for CSHCN (2002), state leaders need other sources for measuring their ongoing progress for achieving the six performance measures. Methods for identifying and assessing state progress are available and a number of states are now engaging in this process. Measuring and monitoring is the fourth step in the systems building process for establishing a system of care for CYSHCN. Measuring & Monitoring Tools |
