Champions Multi-State Meetings

The purpose of the multi-state meetings is to support leaders in their efforts to build community-based systems of services for children and youth with special health needs and their families.

State teams gained experience with a the Champions four-step model for continuous quality improvement (CQI).

  1. Strengthening partnerships with key stakeholders
  2. Developing coordinated plans
  3. Implementing community-based service system strategies
  4. Measuring and monitoring progress

Using this CQI model, state teams developed a leadership in action plan for implementing the six DSCSHN performance measures.

Champions Multi-State Meetings were held in 2004 and 2005.
