Performance Measure 6: Youth with special health care needs will receive the services necessary to make transitions to all aspects of adult life, including adult health care, work, and independence. Why is this important?Youth with special health needs must be able to expect good quality health care, employment with benefits, and independence when they reach adulthood. This requires preparation of youth to take responsibility for their future health and financial needs to begin prior to adolescence so that they will be able to lead productive lives of their choice. Likewise, adult health care services must be available in community settings designed for adults with special needs delivered in culturally effective, and consumer friendly environments.
