New "Focus on Progress" Snapshots Added to Champions Website

Each week more Focus on Progress snapshots are being added to the Champions website. These "snapshots" provide a concise view of each state’s system of services for CYSHCN. Instituting a Medical Home in Your Practice

This article by Dr. Raymond Hubbard provides a pediatrician's perspective on why providing a medical home is essential when caring for Children with Special Health Care Needs (CSHCN). Using experiences with his own practice and within his own state, Dr. Hubbard provides background information on the components of a medical home; providing efficient, quality care; and issues regarding reimbursement. Assistive Technology in Support of Independent Living

"A Day in the Life" of Richard Devylder is a 12 minute on-line video which focuses on the role assistive technology can play in supporting independent living of people with a significant disability. The video also includes a "medical visit" segment, which provides information on how Dr. Devylder can function independently in a "user-friendly" health care setting. Richard Devylder is currently the Deputy Director, External Affairs, at the California Department of Rehabilitation.

Presentations from Transition Seminar Available Online

Presentations from the "Developing a Successful Transition Services Program for Youth with Special Health Care Needs" Seminar, which was held on September 23, 2005 in Jacksonville, Florida, are available on line. There are four presentations and each runs about 30 minutes. This Seminar was presented by Managed Access to Child Health, Inc. (Jacksonville, Florida) and by the Transition Initiative at the University of Florida and was supported by ARC Jacksonville.

Community Voices Program Helps to Improve Health Care Access and Quality

The Kellogg Foundation's Community Voices program has helped 13 communities "create their own networks of affordable services that benefit everyone" without "breaking the bank." Coalitions, including businesses, schools, and citizens' groups, use their "combined buying power to identify and provide the kinds of [health] services that local people ask for."
